Intensive In-Home Qualified Professional (QP), Human Services
Overview
You will own the implementation of person-centered plans for youth and families in the Intensive In-Home program, directly shaping outcomes for up to 15 active cases. You join a clinical team of BCBAs, LCSWs, and senior QPs, supported by weekly supervision. This role differs by pairing crisis intervention with long-term family resource navigation, all within a mission-driven agency.
What You'll Do6
- 1Develop person-centered plans for each youth and family, updating them every 30 days based on progress.
- 2Deliver evidence-based interventions in the home, including behavioral modeling and skill-building sessions.
- 3Provide supportive counseling and crisis intervention, stabilizing families during acute episodes.
- 4Connect families with educational, vocational, and housing resources, maintaining a directory of local providers.
- 5Document all services in the Electronic Health Record within 24 hours, ensuring compliance with state standards.
- 6Participate in weekly team meetings and clinical supervision, presenting case updates and adjusting treatment strategies.
Requirements6
- 1Bachelor's degree in Human Services, Psychology, Social Work, or a related field.
- 22+ years of direct experience with youth or families in a behavioral health setting.
- 3Knowledge of person-centered planning and evidence-based practices (e.g., CBT, ABA).
- 4Valid driver's license and reliable transportation for home visits.
- 5Ability to complete CPR and First Aid certification within 30 days of hire.
- 6Strong written and verbal communication skills for documentation and team collaboration.
