Intensive Care Manager Overflow Gallatin TN - MHC
Overview
You will own care management for children and youth with serious emotional disturbances, coordinating treatment plans and crisis interventions across the Gallatin community. MHC serves Middle and East Tennessee, and you will mentor new care managers while carrying a caseload that demands daily psychiatric, medical, and environmental coordination. You will work with families, schools, and juvenile justice, ensuring seamless transitions from hospital to community. What You'll Do10
- 1Develop individualized service plans and crisis plans with measurable goals for each consumer, updating them as needs change.
- 2Link consumers and families to community resources, advocating for their access to services and benefits.
- 3Ensure consumers are seen within 24-48 hours after hospital discharge, coordinating with Diversion Services to meet Agency standards.
- 4Coordinate daily psychiatric, medical, and environmental needs, arranging appointments and follow-ups.
- 5Mentor new care managers on core skills, including assessment, documentation, and crisis intervention.
- 6Attend morning and weekly team meetings, as well as quarterly All Care Management trainings, and follow up on action items.
- 7Respond to urgent and crisis needs by developing intervention plans with supervisor and crisis services.
- 8Maintain timely and accurate documentation, including progress notes and assessment tools like CANS and Columbia Suicide Risk.
- 9Build relationships with family members, school staff, and juvenile justice contacts to support the consumer's stability.
- 10Meet productivity standards and prioritize a caseload of up to 25 consumers, ensuring quality care within policy guidelines.
Requirements7
- 1Bachelor’s Degree in counseling, psychology, social work, or a related behavioral health field.
- 2Candidates with a criminal justice degree must have 15 college-level semester hours in behavioral health.
- 3Valid Tennessee Driver’s License with an acceptable MVR and criminal background check.
- 4Personal automobile insurance and a reliable vehicle that seats 4 people.
- 5Cell phone for on-call communication.
- 61+ years of experience in behavioral health care management or related field.
- 7Ability to pass a pre-employment drug screen.
