Housing Specialist, Mental Health & Supportive Services
Overview
You will be the primary advocate for adults with mental illness and chronic medical diagnoses, coordinating specialized supports to help them live independently. You will work as part of the MESH team, using Evidence Based Practices like illness management and recovery, motivational interviewing, and WRAP to provide supportive counseling. You will link consumers to medical, social, and vocational services, handle crisis intervention, and document recovery plans. This role demands strong coordination and a commitment to recovery-oriented care.
What You'll Do9
- 1Coordinate each consumer's medical, mental health, social, and vocational services to ensure comprehensive care.
- 2Use Evidence Based Practices including illness management and recovery, motivational interviewing, and WRAP to provide supportive counseling.
- 3Provide crisis intervention for consumers to stabilize and maintain community living.
- 4Assist consumers in building positive relationships with family, neighbors, landlords, and community members.
- 5Help consumers navigate community resources and become familiar with available services.
- 6Facilitate communication between health providers and consumers, including arranging transportation as needed.
- 7Develop recovery goals with consumers and team members, documenting plans in the clinical record within regulatory timeframes.
- 8Document monthly progress and adjustments toward goals, noting interventions and outcomes.
- 9Assist consumers with budgeting and financial management to promote independence.
Requirements4
- 1Bachelor's degree from an accredited university in a mental health related discipline.
- 22+ years of experience in a behavioral health setting.
- 3Ability to be on-call and maintain a flexible work schedule.
- 4Valid driver's license required.
