Grace and Glory Hospice seeks a compassionate Hospice Spiritual Care Counselor to deliver personalized spiritual counseling in EAST BAY. This role provides direct support to patients and families through home visits while collaborating with interdisciplinary teams. Responsibilities include conducting spiritual assessments, developing care plans, and offering bereavement support. What You'll Do11
- 1Conduct spiritual assessments within five days of admission to align care with patient wishes
- 2Develop and implement spiritual components of interdisciplinary care plans
- 3Provide ongoing spiritual support respecting diverse religious traditions
- 4Collaborate with local clergy to meet patient-specific spiritual needs
- 5Maintain detailed clinical documentation including visit notes and interventions
- 6Coordinate scheduling with case managers to ensure holistic care delivery
- 7Participate in IDG and QAPI meetings to enhance spiritual care practices
- 8Lead religious services such as prayer and ritual ceremonies as appropriate
- 9Offer spiritual guidance and bereavement support to patients families
- 10Consult with IDG regarding spiritual care strategies
- 11Organize and execute funeral or memorial services upon request
Requirements11
- 1Ordained commissioned or credentialed religious professional or theology degree
- 2Minimum five years experience supporting patient spiritual needs
- 3Proven ability to work effectively in interfaith environments
- 4Valid driver's license with insurance and reliable transportation
- 5Current TB test results and immunization records
- 6Physical stamina for home visits involving walking stooping and standing
- 7Ability to operate office equipment including computers and multi-line phones
- 8Strong verbal and written communication skills
- 9Licensed driver with proof of insurance
