Homecare Services Coordinator - Asura
Overview
As a Homecare Services Coordinator, you will own the development and management of service delivery plans for participants in Minnesota's CFSS program. You will coordinate with participants, service agencies, and county providers to ensure seamless access to home and community-based services. The role demands strong communication, regulatory navigation, and creative problem-solving. What You'll Do8
- 1Develop and manage individualized service delivery plans based on county assessments, ensuring alignment with CFSS guidelines.
- 2Build participant relationships through ongoing communication, adjusting plans as needs evolve and barriers arise.
- 3Coordinate with Agency and Budget providers, county lead agencies, and DHS to monitor plan success.
- 4Serve as a resource for participants, clarifying policies and procedures related to HCBS, PCA, CFSS, and waivers.
- 5Track service budgets, authorizations, and documentation, ensuring compliance with state and county requirements.
- 6Conduct regular check-ins to address concerns, resolve disputes, and advocate for participant preferences.
- 7Maintain accurate records in electronic documentation systems, supporting audits and reviews.
- 8Stay current on Minnesota regulations and communicate changes to participants and stakeholders.
Requirements8
- 1Bachelor's degree in Human Services, Social Work, Psychology, Healthcare, Rehabilitation, or a related field, OR 1+ years of full-time direct service experience with individuals with disabilities or older adults, OR personal experience coordinating long-term support services.
- 21+ years of experience in case management, care coordination, social services, disability services, or healthcare navigation (preferred).
- 3Knowledge of Minnesota HCBS, PCA, CFSS, waiver programs, or related services.
- 4Experience managing budgets, service plans, or participant-centered programs.
- 5Proficiency with Microsoft Office and electronic documentation systems.
- 6Strong verbal and written communication skills, with the ability to build trust with diverse populations.
- 7Detail-oriented and organized, managing multiple priorities in a dynamic environment.
- 8Self-motivated and capable of working independently while maintaining a participant-centered approach.
