What You'll Do8
- 1Implement care plans under direction of Registered Nurses to deliver high-standard patient care.
- 2Empower patients with preventative and rehabilitative nursing procedures to improve health outcomes.
- 3Collaborate with patients, families, and providers to ensure coordinated, quality care.
- 4Monitor care quality and appropriateness, adjusting as needed.
- 5Follow infection control and organizational safety protocols to maintain a safe environment.
- 6Document patient interactions and care provided using computer systems.

- 8Travel locally to patient homes using reliable transportation.
Requirements6
- 1Graduation from an NLN-accredited LPN program with active New Jersey LPN licensure.
- 22+ years of med/surg or acute care nursing experience.
- 3Home care or hospice experience a plus.
- 4Strong verbal and written communication skills.
- 5Excellent computer skills and organizational abilities.
- 6Valid driver’s license and reliable transportation for local travel.
