What You'll Do8

  • 1Implement care plans under direction of Registered Nurses to deliver high-standard patient care.
  • 2Empower patients with preventative and rehabilitative nursing procedures to improve health outcomes.
  • 3Collaborate with patients, families, and providers to ensure coordinated, quality care.
  • 4Monitor care quality and appropriateness, adjusting as needed.
  • 5Follow infection control and organizational safety protocols to maintain a safe environment.
  • 6Document patient interactions and care provided using computer systems.
  • 7Communicate effectively with team members and patients to foster positive relationships.
  • 8Travel locally to patient homes using reliable transportation.

Requirements6

  • 1Graduation from an NLN-accredited LPN program with active New Jersey LPN licensure.
  • 22+ years of med/surg or acute care nursing experience.
  • 3Home care or hospice experience a plus.
  • 4Strong verbal and written communication skills.
  • 5Excellent computer skills and organizational abilities.
  • 6Valid driver’s license and reliable transportation for local travel.
