What You'll Do7
- 1Monitor vital signs and record patient status changes in medical records using EMR systems.
- 2Deliver wound care and personal care, ensuring patient comfort and safety.
- 3Educate patients and families on care plans and disease management, using Medicare guidelines.
- 4Collaborate with healthcare professionals to coordinate care plans and adjust treatments.
- 5Provide emotional support and companionship to patients, easing end-of-life transitions.

- 7Document all care thoroughly and accurately, meeting HIPAA standards.
Requirements8
- 11+ years of LPN experience in home health or hospice (preferred).
- 2Current LPN license in Pennsylvania.
- 3CPR certification.
- 4Strong communication and organizational skills.
- 5Ability to work independently and as part of a team.
- 6Compassion and emotional resilience for end-of-life care.
- 7Valid driver's license and reliable transportation.
- 8Pass background check and drug screening.
