Home Health Aide HHA - Detroit Michigan
Overview
You will own patient care and outcomes across a full caseload in the Detroit area, setting the clinical standard for an AI-native home healthcare platform. You will deliver hands-on personal care, document in Homecare Homebase (HCHB), and coordinate with supervising nurses and the care team. Your responsiveness and professionalism make every referral source trust us with the next patient. This role stands apart because you will shape the template for future caregivers as the pod grows.
What You'll Do10
- 1Provide personal care assistance including bathing, dressing, grooming, and toileting.
- 2Assist with safe transfers, ambulation, and mobility using proper body mechanics.
- 3Support medication reminders as directed by the care plan.
- 4Monitor and report changes in patient condition, vital signs, and daily living activities to the supervising nurse.
- 5Perform light housekeeping: laundry, meal preparation, and maintaining a clean environment.
- 6Document all care in Homecare Homebase (HCHB) accurately and timely.
- 7Communicate clearly with patients, families, and the care team for continuity of care.
- 8Follow infection control practices and maintain patient dignity and privacy.
- 9Assist with range of motion exercises and restorative care activities as ordered.
- 10Coordinate with supervising RN/LVN for care plan updates and patient needs.
Requirements8
- 1Current Michigan HHA certification
- 21+ years of experience providing personal care (home health, assisted living, nursing home, or hospital setting)
- 3CPR/BLS certification
- 4Valid driver's license and reliable transportation
- 5Ability to lift, transfer, and provide hands-on care safely
- 6Strong communication skills and professional demeanor
- 7Comfortable working independently in patients' homes
- 8High school diploma or GED
