Home Health Aide, Complex Care & ADL Support
Overview
You will provide hands-on complex care for clients in the Maplewood/St. Paul area, covering ADLs, medication reminders, and companionship. Home Health Aide work spans 1-12 hour shifts, including 10 a.m.-6 p.m. and 9 p.m.-12 a.m. You join a Joint Commission Accredited team with RN Case Manager oversight. Spanish-speaking caregivers or those comfortable with Spanish-speaking clients are strongly encouraged. Your work directly improves client quality of life at home.
What You'll Do7
- 1Build rapport through companionship and conversation during visits.
- 2Assist with ADLs: bathing, dressing, grooming, transferring, and ambulation.
- 3Prepare meals and perform light housekeeping to support client independence.
- 4Transport clients to appointments or social outings, ensuring safety.
- 5Monitor and document changes in health or behavior, then report to the RN Case Manager.
- 6Coordinate with the care team to update the plan of care as client needs evolve.
- 7Manage 1-12 hour shifts, including premium pay for short shifts.
Requirements6
- 11+ years in hands-on care: Home Health, Assisted Living, Group Home, or Hospital Setting.
- 2Valid driver’s license, reliable vehicle, and auto insurance.
- 3Negative TB test or cleared chest X-ray.
- 4Availability for 2 weekend days per month.
- 5Compassionate, patient, and caring nature with strong communication skills.
- 6Comfort working with Spanish-speaking clients (bilingual a plus).
