What You'll Do10
- 1Perform Start of Care patient visits including initial assessments and medication reconciliation
- 2Create personalized care plans to improve patient outcomes
- 3Conduct wound assessments and provide wound care treatments
- 4Administer and monitor IV/infusion therapies per physician orders
- 5Complete accurate clinical OASIS documentation
- 6Act as trusted case manager coordinating care with physicians and therapists
- 7Educate patients and caregivers with confidence

Requirements10
- 1Currently licensed as a Registered Nurse in Arizona
- 2Minimum of 1 year Home Health experience
- 3Wound care and/or infusion experience preferred
- 4Strong clinical skills and passion for patient-centered care
- 5Expert organizational and time management skills
- 6Current CPR certification
- 7Valid AZ driver's license and proof of auto insurance
- 8Reliable transportation
