What You'll Do9
- 1Accept and treat patients within designated geographic area
- 2Evaluate patient needs and develop individualized treatment plans
- 3Create plan of care and set treatment goals with physician family
- 4Implement customized therapy programs to restore function
- 5Instruct family members and aides on therapy techniques
- 6Prepare and document visits accurately
- 7Communicate with patients families and interdisciplinary team
- 8Deliver care complying with agency policies
- 9Travel to patient homes within assigned territory
Requirements5
- 1Licensed Physical Therapist program graduate
- 2New York state license or eligibility
- 3First Aid AED CPR certifications
- 4Home health experience preferred
- 5Mandatory certifications as listed
