What You'll Do11

- 4Follow up high-utilizer patients post-hospitalization
- 5Coordinate durable medical equipment logistics
- 6Answer clinic calls with specialized service knowledge
- 7Research and expand community resource networks
- 8Facilitate patient group activities in social spaces
- 9Identify creative support methods for complex needs
- 10Maintain accurate administrative records related to social determinants
- 11Drive outreach initiatives for transportation and food access
Requirements10
- 1Minimum one year experience in medical or community service settings
- 2One-on-one patient interaction expertise
- 3Frequent phone and in-person communication abilities
- 4Proficiency with email and calendar systems
- 5Strong English language skills
- 6Regular office availability Monday-Friday
- 7US employment eligibility
- 8Associates or bachelor's degree preference
- 9Birth or end-of-life doula experience
