Direct Support Professional, Mental Health & Community Living Supports
Overview
Direct Support Professional role supporting individuals with disabilities in community living settings. You will assist with ambulation, personal care, meal planning, light housekeeping, and daily living skills at the client’s home or in the community. Work with a client care team to review CSSPs, set goals, and develop action plans to enhance independence. This role emphasizes hands-on training, caregiver collaboration, and practical strategies for daily living. You’ll build lasting relationships and help clients navigate community resources at scale.
What You'll Do10
- 1Build plans and provide support based on CSSPs to help clients achieve daily living goals
- 2Collaborate with clients and support team members to deliver person-centered care
- 3Drive clients to community outings, appointments, and errands
- 4Provide supervision and training for routine household tasks and personal care
- 5Assist clients with meal planning, budgeting, and basic paperwork
- 6Assess risks and mitigate them while supporting client safety and independence
- 7Guide clients through activities of daily living and behavior management
- 8Help clients access community resources and scheduling
- 9Support clients in developing skills like cooking, budgeting, and time management
- 10Document progress and outcomes to inform ongoing care plans
Requirements10
- 1Must be 18 years or older
- 21-2 years of supervised experience with individuals with disabilities and/or challenging behaviors or relevant degree (Psychology, Social Work, Human Services)
- 3Strong interpersonal skills and ability to respect boundaries
- 4Reliable transportation and valid driver’s license with proof of car insurance
- 5Knowledge of local community resources is a plus
- 6Pass Minnesota Department of Human Services Background Study
- 7Comfortable visiting clients in home or community settings
- 8Friendly, outgoing demeanor

- 10Ability to work onsite in St Paul area and participate in a team-oriented environment
