Dialysis Chronic Care Registered Nurse
Overview
Dialysis Chronic Care Registered Nurse leads patient care for ESKD populations. Responsible for assessment planning and delivery of hemodialysis therapy while collaborating with interdisciplinary teams. Requires 12 months dialysis experience and strong clinical expertise.
What You'll Do11
- 1Perform systematic patient data analysis before during and after hemodialysis treatments and document findings in medical records
- 2Assess patient learning needs and provide education to patients and families
- 3Direct daily hemodialysis delivery with RNs LPNs and technicians under supervision
- 4Administer medications according to protocols and document effectiveness
- 5Initiate emergency response measures when needed
- 6Serve as resource for healthcare team and participate in staff training
- 7Ensure proper laboratory collection shipping and track missed collections
- 8Identify patient outcomes and update nursing plans through interdisciplinary collaboration
- 9Discuss transplant options and treatment modalities with patients
- 10Maintain accurate documentation and ensure compliance with regulations
- 11Travel to training sites as required while maintaining physical demands
Requirements10
- 15+ years dialysis experience with chronic kidney disease patients
- 23 months acute hemodialysis experience required
- 3Graduate from accredited nursing school
- 4Current state licensure and CPR BLS certification
- 5Minimum 9 months RN licensure
- 6Floating capability to work across multiple sites within 50 mile radius
- 7Proficiency with AWS Certified solutions
- 8Expertise in Python and React frameworks
- 9Certification in CPC Card management
- 10Skills in Kubernetes orchestration
