Overview

Own the delivery of community based education and family support for children from birth to age five across Denver Metro partner sites. Lead developmentally appropriate activities, complete ongoing assessments, and maintain documentation to track child progress. Collaborate with families, educators, and community partners to promote school readiness and family well being. This role emphasizes hands on engagement, case management, and crisis support in a community setting.

What You'll Do10

  • 1Build and implement programs to meet student needs and involve parents in development
  • 2Design weekly plans from ongoing assessments to guide instruction
  • 3Lead case management and family goal setting with progress monitoring
  • 4Coordinate family participation in meetings, events, and volunteer activities
  • 5Conduct home visits and align services with internal stakeholders to support each student
  • 6Create tools and reports for families and internal teams and maintain accurate documentation
  • 7Collaborate with health service staff to ensure health requirements are met and referrals are made
  • 8Ensure attendance targets are met by partnering with families and schools
  • 9Provide crisis intervention and support for issues such as child abuse, domestic violence or housing needs
  • 10Support classroom coverage and behavior planning with families as needed

Requirements10

  • 1Bachelor's degree in Social Work, Human Family Development, or related field
  • 21+ years of experience in child facing roles
  • 3CPR Certification
  • 4Pediatric First Aid Certification
  • 5Universal Precautions Certification
  • 6Valid driver license with auto insurance
  • 7Colorado background checks including CBI, FBI and National Sex Offender Registry
  • 8Bilingual preferred
  • 9Strong communication and intercultural skills
  • 10Proficiency with Microsoft Office Suite and teaching software
