Overview

Per Diem pharmacist provides professional level functions to assist in providing patients with optimum pharmaceutical care. The pharmacist reviews medication orders conducts interventions with nurses and physicians oversees technicians and participates in quality improvement efforts.

What You'll Do5

  • 1Review medication orders and oversee dispensing of medications and pharmaceutical preparations
  • 2Conduct interventions with nurses and physicians and prepare continuous narcotic infusions on nursing units
  • 3Evaluate non-formulary drug orders and recommend therapeutic alternatives
  • 4Interview patients to assess adverse drug reactions and document ADR information
  • 5Participate in medication reconciliation processes

Requirements3

  • 1Bachelor's degree or Pharm D required
  • 2Pharmacy license required
  • 31 + years of hospital pharmacy experience preferred
