Overview

Join Fairview Health Services as an In Clinic Visit Facilitator for our Pediatric Care team, owning the patient experience from pre-visit communication to post-visit wrap-up. You will guide families through each appointment, collect and document vital medical data, and ensure providers have what they need for seamless care. This role operates within a supportive team environment at our Riverside Clinic, with no weekends required. You will directly impact pediatric patient satisfaction and clinic efficiency, backed by robust Fairview resources and a structured 0.8 FTE schedule.

What You'll Do6

  • 1Prepare patients for appointments by calling ahead to collect medical records and confirm availability.
  • 2Room patients at visit time, capturing reason for visit, medication reviews, and vitals via automated equipment.
  • 3Stock exam rooms with supplies and ensure equipment readiness for each provider.
  • 4Connect patients to providers, acting as a resource for a positive experience.
  • 5Complete visit wrap-up, providing after-visit summaries and scheduling follow-ups.
  • 6Communicate with patients, providers, and staff using in-person and virtual technologies.

Requirements6

  • 11 year of customer service experience in a healthcare or service setting.
  • 2Proficiency with technology, including computer hardware, software, and smart devices.
  • 3Preferred: Current student in nursing, medical, social work, psychology, or pharmacy.
  • 4Preferred: Completion of Nursing Assistant, EMT, or Health Unit Coordinator program.
  • 5Preferred: Knowledge of medical terminology and Epic health records.
  • 6Preferred: Telephonic customer experience and prior healthcare setting work.
