Client Care Coordinator, Senior Home Care
Overview
You own the client journey from first inquiry to ongoing care coordination, managing a portfolio of 30-50 senior clients and their families. You work on a team of 5 Care Coordinators and 2 Care Managers, reporting to the Director of Client Services. This role stands out for its local travel to client homes and senior living communities, offering a front-line view of care and a direct impact on quality of life.
What You'll Do8
- 1Build and maintain trust-based relationships with clients and families, serving as their primary point of contact.
- 2Conduct in-home Care Consultations and quarterly reassessments to evaluate physical, cognitive, and social needs.
- 3Develop and update personalized Plans of Care within 48 hours of any change in client status.
- 4Coordinate with Care Professionals and the office team to ensure seamless daily care delivery.
- 5Identify evolving client needs and recommend new services or adjustments, such as introducing Alzheimer's care or diabetes management.
- 6Resolve client concerns with empathy and professionalism, escalating to the Director when needed.
- 7Maintain accurate client documentation in Wellsky and Home Care Pulse, adhering to state and company standards.
- 8Visit clients in senior living communities and private homes across Wilmington and Newark, covering up to 20 miles per day.
Requirements7
- 12+ years in a client-facing role in home care, healthcare, social work, or senior services.
- 21+ year of experience in care coordination or case management.
- 3Proven ability to build relationships and communicate with empathy in sensitive situations.
- 4Strong organizational skills with the ability to prioritize when managing multiple clients and schedules.
- 5Comfort with Microsoft Office, Google Workspace, and electronic health records.
- 6Valid driver's license and reliable transportation for local travel to client homes.
- 7Ability to work independently and as part of a team in a dynamic environment.
