Case Manager Registered Nurse
Overview
Registered Nurse Case Manager leading pediatric private duty nursing across Central Texas. Oversee large service area while collaborating with families clinicians and internal leadership to drive patient outcomes and market expansion. Territory spans Hillsboro to Kyle and Bryan to Waco.
What You'll Do11
- 1Manage pediatric PDN caseload throughout assigned territory
- 2Coordinate patient care as primary clinical resource for families and field clinicians
- 3Conduct supervisory visits patient assessments and skilled nursing visits as needed
- 4Monitor progress and collaborate with physicians families and interdisciplinary teams
- 5Complete QA reviews to ensure documentation accuracy and regulatory compliance
- 6Maintain complete electronic medical records
- 7Interview candidates for prospective nursing staff and support recruitment
- 8Partner with scheduling and talent acquisition teams for clinician placement
- 9Support referral development and market growth through physician hospital and community partnerships
- 10Represent Amazing Care at community events networking opportunities and career fairs
- 11Collaborate with Director of Nursing and leadership on operational initiatives and quality improvement
Requirements11
- 1Current Texas Registered Nurse license in good standing
- 2Bilingual Spanish proficiency
- 3Minimum one year nursing experience
- 4Pediatric home health or private duty nursing experience preferred
- 5Previous case management or leadership experience
- 6Strong clinical assessment and critical thinking skills
- 7Excellent communication and organizational abilities
- 8Proficiency with EMR systems
- 9Valid Texas driver’s license reliable transportation current auto insurance
