Case Manager, HomeCare Services & Community Outreach
Overview
Own client care coordination at scale across Delaware County. Lead assessments, documentation, and caregiver support to ensure compassionate, compliant home care. Collaborate with clinicians, scheduling, and referral partners to deliver seamless transitions and high-touch service. This role stands out through proactive client engagement and local community presence.
What You'll Do10
- 1Build initial client introductions with new caregivers to ensure smooth transitions
- 2Design and complete reassessments and updates as client conditions change
- 3Maintain regular client high-touch communications through calls and visits
- 4Host post-hospitalization follow-ups to ensure safe transitions home
- 5Coordinate caregiver schedules with Scheduling and Care Team leadership
- 6Assist with caregiver orientation and participation in LEAP meetings
- 7Provide coaching and problem-solving support to caregivers in the field
- 8Audit care logs daily for compliance and report unsigned items to billing
- 9Maintain accurate client records in OneDrive and ensure regulatory compliance
- 10Represent HomeWell at community events and referral partnerships
Requirements10
- 1High school diploma or GED required
- 2Associate's or Bachelor's degree in Healthcare, Human Services, Social Work, Nursing, or related field preferred
- 3Minimum of two (2) years of healthcare, home care, case management, or caregiving experience
- 4Experience coordinating client care preferred
- 5Experience with electronic medical records (WellSky preferred)
- 6Excellent customer service and communication skills
- 7Ability to manage multiple priorities and strong organizational skills
- 8Problem-solving, conflict resolution abilities, and professional judgment
- 9Proficiency with Microsoft Office, OneDrive, and electronic health records
- 10Ability to travel daily within service area and lift up to 25 pounds
