Care Manager Social Worker AFC
Overview
We seek a dedicated professional to lead psychosocial assessments and care management for adult foster care clients. This full-time position involves direct client interaction and coordination within Massachusetts.
What You'll Do11
- 1Conduct client assessments and care management
- 2Complete AFC-required assessments and documentation
- 3Maintain accurate client charts and records
- 4Support individualized care plans through social and emotional interventions
- 5Perform home evaluations to verify AFC-qualified settings
- 6Collaborate with nurses to select and train caregivers
- 7Provide caregiver education and ongoing support
- 8Connect families with community resources and benefits
- 9Coordinate care across providers and agencies
- 10Ensure compliance with state regulations and program standards
- 11Deliver compassionate assistance to improve client outcomes
Requirements10
- 1Bachelor’s degree in Social Work or related field
- 2Licensed Social Worker in Massachusetts or bachelor’s degree with two years clinical experience
- 3Two years experience serving elderly or disabled populations
- 4Strong documentation and communication abilities
- 5Knowledge of Massachusetts community resources
- 6Valid driver’s license and reliable vehicle
- 7Proficiency with electronic health records
- 8Experience with AFC/GAFC or Adult Day Health programs
- 9Care coordination or case management background
- 10Ability to travel locally within Boston area
