What You'll Do5
- 1Build and maintain care plans with the IDT to ensure seamless post discharge services
- 2Coordinate admission process and update care plans to reflect patient needs
- 3Attend and contribute to daily IDT meetings with clinical reviews as needed
- 4Complete sections of Discharge Plan Instructions and review post discharge instructions with guests
- 5Review treatment effectiveness and document outcomes to support continuous improvement
Requirements5
- 1Must have a Nursing Degree from an accredited program or be a graduate of an approved LPN/LVN program
- 2Must hold an active RN or LPN/LVN license in good standing with the State Board
- 3Minimum of two years experience in hospital, nursing facility, or related health care setting
- 4Ability to collaborate with interdisciplinary teams and participate in care coordination
- 5Willingness to work onsite in Scottsdale, AZ and support patient-centered care
