What You'll Do8
- 1Provide direct patient care as an LPN within the facility
- 2Document patient status and communicate changes to the care team
- 3Support medication administration and treatment plans
- 4Assist with daily living activities and comfort measures for residents
- 5Contribute to a positive culture and compliance with survey readiness
- 6Support continuity of care and accurate charting in PointClickCare
- 7Collaborate with licensed staff to ensure safe, effective care
- 8Participate in rounds and care planning to improve patient outcomes
Requirements7
- 1LPN license (Required)
- 2CRP certification (Must)
- 3Finger Print Card qualification (Must)
- 4Experience with PointClickCare (Preferred 1 year)
- 5Skilled nursing facility experience (Required 1 year)
- 6Ability to commute to Glendale, AZ 85302 (Required)
- 7Ability to relocate to Glendale, AZ 85302 before starting (Required)
