Care Management Program Manager, ECM & Health Equity

Own the Enhanced Care Management program at scale across California, leading regional teams to deliver high-quality, person-centered care for high-need populations. You’ll oversee program quality, member services, partnerships, budgeting, and growth initiatives to close health equity gaps. What You'll Do10
- 1Lead and mentor Lead Care Managers across multiple counties and conduct weekly coaching sessions
- 2Set program benchmarks, implement QA protocols, and use data to identify trends and drive improvements
- 3Oversee member services to ensure in-person visits and high-quality care and improve engagement
- 4Coordinate with hospitals, providers, and community organizations to optimize care transitions
- 5Manage budgets and resources, adjusting staffing based on member needs and priorities
- 6Identify opportunities to enhance program effectiveness and pilot initiatives such as telehealth
- 7Lead operational huddles and bi-weekly case conferences and develop regional trainings
- 8Prepare quarterly performance reports and maintain stakeholder relationships
- 9Monitor documentation and support challenging cases while ensuring regulatory compliance
- 10Maintain strong leadership presence with regular in-person staff meetings and site visits
Requirements10
- 1Bachelor’s or Master’s in Healthcare Administration, Public Health, Nursing, or related field
- 25+ years in healthcare management including 3+ years in care or case management
- 32+ years managing Enhanced Care Management or intensive care management programs
- 4Understanding of care management principles and healthcare delivery systems
- 5Experience with Medicaid/Medicare programs and ECM program requirements
- 6Strong analytical skills and experience in data-driven program evaluation
- 7Excellent leadership and communication skills
- 8Ability to manage multiple priorities in a fast-paced environment
- 9Commitment to person-centered care and health equity
- 10Willingness to travel regularly to county sites in California
