Care Manager II (Russian Speaking) - CalAIM ECM
Overview
You will own care coordination for high-need Medi-Cal members in Santa Clara county, with a focus on housing stability and whole-person health. Working under a Clinical Supervisor at Jewish Family Services of Silicon Valley, you will join a team that uses CalAIM funding to deliver person-centered care. This role stands out for its emphasis on in-person client engagement and Russian-speaking cultural competence in a diverse community.
What You'll Do10
- 1Build person-centered care plans that address medical, behavioral, and social needs, with a focus on safe, sustainable housing solutions.
- 2Navigate clients through physical and behavioral health systems, coordinating with providers and community services per the care plan.
- 3Conduct regular in-person visits to assess client needs, provide support, and ensure effective delivery of care services.
- 4Educate clients, families, and support systems about their condition and connect them to resources that support health goals.
- 5Identify and allocate community and social support services to meet unique client and family needs.
- 6Advocate for client rights, preferences, and best interests throughout the care journey.
- 7Act as the central communication hub, ensuring smooth information exchange among clients, families, and providers.
- 8Document assessments, care plans, and progress notes in detail, adhering to legal and organizational standards.
- 9Stay current on Medi-Cal and CalAIM regulations, ensuring compliance in all care delivery.
- 10Integrate JFS values (Welcoming the Stranger, Repairing the World, Respect and Dignity) into daily work.
Requirements10
- 1Master's degree in Social Work (MSW) required.
- 22+ years of direct Enhanced Care Management (ECM) experience required.
- 3Strong knowledge of social determinants of health and community resources.
- 4Familiarity with Medi-Cal, CalAIM, and the 14 Community Supports services preferred.
- 5Experience with databases, specifically Apricot, highly desirable.
- 6Cultural competence and sensitivity to diverse populations.
- 7Proficiency in Motivational Interviewing and needs assessment.
- 8Excellent interpersonal, communication, and service coordination skills.
- 9Strong organizational, time management, and data entry abilities.
- 10Russian-speaking preferred.
