Overview
Join GroundGame.Health as a Lead Care Manager to bridge health equity gaps by integrating social care into clinical settings. You will lead a multidisciplinary team to identify and address Health-Related Social Needs and Social Determinants of Health for members enrolled in ECM CHI CHW programs. This role combines trauma-informed care with innovative technology to ensure equitable, high-quality outcomes.
What You'll Do11
- 1Manage patient assignments and provide coaching and case consultation to a team of Community Health Workers
- 2Conduct comprehensive HRSN and SDOH assessments and develop individualized care plans in collaboration with members
- 3Coordinate referrals and monitor outcomes to ensure timely resolution of identified needs
- 4Perform telephonic clinic and community outreach to support member engagement and program participation
- 5Review documentation for quality accuracy and ensure proper billing in EHR and Implify platforms
- 6Monitor team productivity and support staff development and workflow improvements
- 7Maintain HIPAA compliant documentation of member interactions and care plans
- 8Participate in interdisciplinary care team meetings and serve as operational liaison between provider practices and GroundGame.Health
- 9Ensure adherence to provider dress codes and safety protocols including TB testing and PPE requirements
- 10Support implementation of care management initiatives and process improvements
- 11Provide leadership and remote oversight to CHWs while meeting contractual obligations
Requirements9
- 1Community Health Worker Certification or 2+ years experience in care coordination case management
- 2Proficiency with EMRs and familiarity with Implify platform
- 3Valid driver’s license and reliable transportation to visit patients and provider locations
- 4Bilingual English and Spanish communication skills
- 5Ability to work onsite in Huntington Park CA and surrounding areas
- 6Strong understanding of Health-Related Social Needs and Social Determinants of Health frameworks
- 7Bachelor’s degree preferred in relevant fields such as Social Work or Public Health
- 8Certification in Community Health Worker recognized by state or national board
- 9Experience supporting CalAIM Enhanced Care Management and Community Health Integration programs
