Overview

Coordinate the full admissions and discharge cycle for clients at the Lighthouse for Women program, a substance use disorder treatment center. Own intake assessments, bed assignments, and discharge planning, working with a small clinical team. This role requires strict adherence to confidentiality laws like HIPAA and direct collaboration with referral sources and aftercare providers. You will make a direct impact on client continuity of care from first contact through aftercare.

What You'll Do16

  • 1Take referrals and complete intake paperwork for new clients.
  • 2Conduct interviews with patient referrals to assess program fit and appropriateness.
  • 3Complete admission assessments for new patients as needed.
  • 4Present admission information to the Treatment Team for approval.
  • 5Maintain contact with waiting list clients and referral sources.
  • 6Update assessments and complete admission paperwork when patients arrive.
  • 7Schedule admissions and maintain updated bed assignment and waiting lists.
  • 8Send daily census reports as directed.
  • 9Participate in Clinical Team meetings and report daily progress of social service activities.
  • 10Ensure transportation for scheduled admissions and maintain accurate documentation of patient contacts and discharge planning.
  • 11Complete referrals to housing programs per therapist's recommendation.
  • 12Arrange aftercare appointments for both CD and MH providers and coordinate patient services with outside agencies (domestic violence, trauma, etc.).
  • 13Assist patients in securing safe housing by arranging halfway or 3/4 house placement interviews.
  • 14Assist patients in completing the Discharge Planning Guide and Continuing Care Plan.
  • 15Facilitate Continuing Care group and other social services groups as assigned.
  • 16Coordinate patient in/out flow and perform other duties as assigned.

Requirements7

  • 1High school diploma or GED with some experience in a human service setting.
  • 2Pass PA Criminal Background Check, PA Child Abuse and FBI Fingerprinting clearances.
  • 3Pass drug screen and 2-Step TB Test.
  • 4Familiarity with electronic health records (EHR) and other administrative tools.
  • 5Understanding of ethical issues related to treatment, consent, and client rights.
  • 6Understands confidentiality laws, such as HIPAA, and the importance of maintaining client privacy.
  • 7Ability to develop and maintain relationships with referral sources and follow-up services.
