Admissions Discharge Coordinator, Behavioral Health Intake
Overview
You will own the full patient intake and discharge cycle at Gateway Rehab Center's Lighthouse for Women program in Washington, PA. You coordinate referrals, admissions, and aftercare planning for women in substance use disorder treatment. You work with the clinical team, outside agencies, and housing programs to ensure smooth transitions. What You'll Do11
- 1Take referrals, complete intake paperwork, and conduct patient interviews to assess program fit.
- 2Complete admission assessments and present findings to the Treatment Team for approval.
- 3Maintain the waiting list, assign beds, and schedule admissions to keep census accurate.
- 4Send the daily census report and update patient records in the EHR system.
- 5Coordinate transportation for scheduled admissions and arrange aftercare with CD and MH providers.
- 6Facilitate Continuing Care group and other social services groups as assigned.
- 7Complete referrals to housing programs and arrange halfway or 3/4 house placement interviews.
- 8Assist patients in completing the Discharge Planning Guide and Continuing Care Plan.
- 9Coordinate patient in/out flow and maintain contact with referral sources.
- 10Attend mandatory Gateway trainings and in-services.
- 11All other duties assigned by the Program Supervisor or Administrator.
Requirements7
- 1High school diploma or GED with some experience in a human service setting.
- 2Pass PA Criminal Background Check, PA Child Abuse and FBI Fingerprinting clearances.
- 3Pass drug screen and 2-Step TB Test.
- 4Familiarity with electronic health records (EHR) and other administrative tools.
- 5Understanding of ethical issues related to treatment, consent, and client rights.
- 6Knowledge of confidentiality laws, such as HIPAA, and commitment to client privacy.
- 7Ability to develop and maintain relationships with referral sources and follow-up services.
