Overview
You will own the 245D compliance program for Blue Talent Development, ensuring full licensing standards for services to adults with disabilities. You will coordinate referrals with case managers and counties, build community partnerships, and safeguard individual rights at a 245D-licensed agency. This part-time contract role offers a flexible schedule and professional development support.
What You'll Do9
- 1Maintain current knowledge of Minnesota DHS 245D licensing requirements and ensure the agency complies with all applicable rules.
- 2Oversee the Designated Coordinator duties, ensuring they meet 245D standards for service delivery and documentation.
- 3Drive program oversight, implementing policies and procedures that protect the rights, dignity, and choices of each person served.
- 4Complete internal reviews of alleged maltreatment incidents per 245D requirements and document findings.
- 5Measure satisfaction of individuals, legal representatives, and case managers through surveys and feedback sessions.
- 6Guide staff in maintaining compliance with agency policies and 245D licensing standards through trainings and audits.
- 7Build referral pipelines by responding to inquiries and coordinating intake, assessments, and admissions with internal teams.
- 8Forge partnerships with county agencies, social workers, and healthcare providers to connect eligible adults to services.
- 9Represent the agency at networking events and community meetings, promoting services and referral pathways.
Requirements9
- 1Bachelor's degree in human services or related field plus 3+ years of full-time supervisory experience in direct-care for persons with disabilities or age 65+.
- 2Associate degree in human services or related field plus 3+ years of full-time supervisory experience in direct-care for persons with disabilities or age 65+.
- 3Diploma in human services from an accredited institution plus 3+ years of full-time direct-care experience and 3+ years of supervisory experience.
- 450 hours of education/training in human services and disabilities plus 4+ years of full-time direct-care experience under a qualified supervisor.
- 5Experience with Minnesota DHS 245D-licensed programs and knowledge of 245D requirements.
- 6Proven ability to communicate with case managers, counties, and healthcare providers.
- 7Strong organizational skills for managing referrals, intake, and community outreach.
- 8Demonstrated understanding of person-centered practices and individual rights.
- 9Ability to work independently and manage multiple priorities effectively.
