Weekend Registered Nurse, Home Health
Overview
You will own the full nursing care plan for home health patients in the Springhill, TN area, from admission through discharge, at a scale of 15-20 visits weekly. You will coordinate with physicians, families, and agency staff to prevent re-hospitalization and promote wellness. What You'll Do8
- 1Build and implement individualized care plans for home health patients, ensuring compliance with physician orders and state regulations.
- 2Own the initial and comprehensive nursing evaluation for new patients, determining eligibility and medical necessity per payer guidelines.
- 3Drive continuity of care by coordinating with physicians, agency personnel, patients, and families on a weekly basis.
- 4Document patient conditions, goals, and interventions in clinical notes within 24 hours of each visit.
- 5Coordinate visit routes to maximize efficiency and ensure timely care for all assigned patients.
- 6Deliver high-quality clinical services within your scope of practice, from admit through discharge.
- 7Adjust care plans based on patient response and changing needs, communicating updates to the care team.
- 8Direct patient education on disease management and medication to reduce re-hospitalization.
Requirements5
- 11+ year of clinical RN experience.
- 2Current RN license in the state where services are rendered.
- 3Current CPR certification.
- 4Valid driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
- 5Ability to commute to multiple patient residences daily within the assigned geographic area.
