Transitional Care Patient Care Technician
Overview
Functions as part of the dialysis health care team in providing safe and effective home dialysis therapy and training for patients under the direct supervision of a licensed nurse. Provides support for quality enhancement initiatives and patient satisfaction.
What You'll Do15
- 1Reinforces home hemodialysis training and peritoneal dialysis training as permitted by law
- 2Identifies and welcomes patients obtains vital signs weights and gathers information for intake forms
- 3Builds communication and collaboration processes with patients care partners and clinical staff
- 4Reports missing or abnormal data from treatment records to the registered nurse
- 5Assists with electronic home treatment reconciliation and tracks shipping of specimens
- 6Prepares supplies for laboratory testing and manages inventory to reduce waste
- 7Schedules patient appointments and coordinates reminders with caregivers
- 8Disinfects dialysis equipment between patient visits and maintains clean treatment areas
- 9Sets up trains treats and troubleshoots home hemodialysis machines
- 10Evaluates dialysis access before treatment and documents findings
- 11Initiates and monitors home dialysis treatments while documenting patient status
- 12Performs safety checks and reports significant changes in patient condition
- 13Collects and documents post-treatment vital signs
- 14Assists with water and dialysate testing as directed
- 15Supports quarterly water testing and maintains supply organization
Requirements10
- 15+ years building ETL pipelines with Spark and Airflow
- 23-5 seasons of dialysis patient care experience
- 3CPC Card certification
- 4BCBA certification
- 5AWS Certified experience
- 6High School diploma or GED
- 7State licensure and current certifications
- 8CPR certification
- 9Ability to pass Ishihara color blindness test
- 10Demonstrated commitment to patient care and organizational mission
