Spring Clinical Pharmacist Infusion Therapy Contract
Overview
The RPh on the Go seeks a clinical pharmacist to lead infusion therapy in Spring TX for 8-12 weeks. This role involves preparing IV and subcutaneous meds in home settings while collaborating with a multidisciplinary team. Candidates must ensure therapy effectiveness patient education and regulatory compliance.
What You'll Do7
- 1Prepare specialty pharmaceuticals and IV solutions under sterile conditions following state and federal regulations
- 2Monitor patient lab values calculate pharmacokinetic dosing such as vancomycin and adjust Total Parenteral Nutrition regimens
- 3Provide detailed education and counseling to patients and caregivers on infusion pumps sterile techniques and catheter care
- 4Coordinate care with physicians and home health nurses to optimize treatment outcomes and reduce hospital readmissions
- 5Collaborate with intake referral reimbursement nursing and pharmacy teams to support patient transitions
- 6Ensure compliance with regulatory standards including State Board of Pharmacy HIPAA FDA DEA Medicare ACHC URAC and OSHA
- 7Manage pharmacy inventory including refrigerated products and oversee packing and shipping of medications to patient homes
Requirements6
- 15+ years building ETL pipelines with Spark and Airflow
- 2Texas pharmacist license in good standing
- 31-3 years of experience in infusion specialty pharmacy or hospital pharmacy settings
- 4Familiarity with IVIg therapy is a plus
- 5Understanding of specialty therapy payer requirements reimbursement procedures and contract obligations
- 6Proficiency with CareTend system
