What You'll Do11
- 1Build discharge planning evaluations and coordinated care pathways for inpatient observation and emergency departments
- 2Lead patient and family care conferences with multidisciplinary teams to organize transitions of care
- 3Assess patient and family needs across clinical social and financial domains to prevent discharge delays
- 4Document comprehensive discharge plans incorporating medical records and develop contingency strategies
- 5Review medical histories and coordinate community resources for social needs alongside clinical care
- 6Assess readmission factors through patient and family interviews and facilitate care coordination
- 7Facilitate patient-centered communication across diverse populations with adaptability and cultural competence
- 8Maintain accurate documentation and manage electronic health records in compliance with standards
- 9Collaborate with licensed professionals to implement evidence-based interventions
- 10Advocate for patient rights and support choices during hospitalization
- 11Develop whole-person wellness strategies addressing medical psychological and spiritual dimensions
Requirements5
- 1Excellent interpersonal communication negotiation critical thinking organizational skills and cultural adaptability require bachelor's degree preferred master's degree field of study social work
- 2Minimum one year social work experience required accredited case manager or certified case manager preferred
- 3Physical ability to perform light lifting and stand for extended periods
- 4Valid Florida license and case management certification
- 5Background screening clearance as mandated by Florida law
