Requirements10
- 1Graduate from Accredited Nursing School with 5+ years of SNF experience
- 2Active RN license with valid certification
- 3Proven track record in care coordination and case management
- 4Certified in CPC or BCBA
- 5Experience with AWS and Kubernetes platforms
- 6Strong knowledge of value-based care models
- 7Ability to travel frequently across Phoenix metropolitan area
- 8Physical stamina for standing and walking throughout workday
- 9Experience with EHR systems and data analytics tools
