Overview
RN responsible for delivering in home skilled nursing care to patients managing chronic and acute conditions while promoting independence and safety at scale. Collaborates with physicians and home health disciplines to coordinate care and ensure positive outcomes. What You'll Do6
- 1Build comprehensive in home assessments and OASIS documentation where applicable to drive accurate care planning.
- 2Develop and update individualized plans of care in collaboration with physicians and the home health team.
- 3Administer medications, wound care, and other skilled nursing interventions in patients' homes to achieve targeted outcomes.
- 4Educate patients and caregivers on disease management, medications, and safety to enhance adherence.
- 5Coordinate care with physical therapy, social work, and other home health disciplines to optimize patient function and support.
- 6Document visits, assessments, and patient progress in compliance with regulatory requirements to ensure audit readiness.
Requirements5
- 1Associate's or Bachelor's Degree in Nursing (Required)
- 2Minimum 1-2 years of med-surg, home health, or community health nursing experience (Preferred)
- 3Current RN license in state of practice (Required)
- 4Basic Life Support (BLS) certification (Required)
- 5Valid driver's license and reliable transportation (Required)
