Registered Nurse HHD Intermediate
Overview
DaVita seeks an organized self‑directed RN to manage Home Hemodialysis patients and empower caregivers to perform dialysis independently at home. This role combines clinical care patient education and case management. The ideal candidate will train individuals on HHD procedures monitor treatment progress conduct home visits and collaborate with interdisciplinary teams. Monday Friday shifts with flexible scheduling.
What You'll Do5
- 1Train patients and caregivers on HHD procedures and self‑care
- 2Manage a caseload of HHD patients including assessments and care planning
- 3Monitor treatment progress conduct home visits and provide on‑call support
- 4Collaborate with interdisciplinary teams to ensure safe high‑quality outcomes
- 5Schedule Monday–Friday 8‑hour shifts weekends and holidays off
Requirements5
- 1RN license in the state of practice
- 2Valid driver’s license insurance and reliable transportation
- 3ADN required BSN preferred
- 4Basic computer skills MS Word Outlook
- 5Preferred experience dialysis HHD PD chronic or acute patient education or teaching background case management or telehealth experience CNN or CDN certification
