Recovery Specialist Case Management - Contract Role
Overview
Own the full client case management cycle for a 13-week contract at a leading healthcare facility, guiding individuals through substance abuse recovery. You will conduct intake assessments, develop treatment plans, and coordinate aftercare. What You'll Do8
- 1Build rapport with clients during intake assessments to identify treatment needs and personal goals.
- 2Design individualized treatment plans aligned with recovery milestones and clinical guidelines.
- 3Coordinate referrals to community resources for aftercare and housing support.
- 4Document client progress and treatment compliance with accuracy and timeliness.
- 5Administer medication management and coordinate transportation to appointments.
- 6Intervene during crises using de-escalation techniques to ensure safety and stability.
- 7Facilitate group education sessions on wellness and relapse prevention.
- 8Partner with clients, families, and healthcare providers to adjust care plans as needed.
Requirements8
- 12-3 years experience in chemical dependency or substance abuse treatment.
- 2Bachelor's degree in Human Services or related field; Associate degree considered.
- 3Certified Addiction Counselor (CAC) certification preferred.
- 4CPI and CPR certification required during onboarding.
- 5BLS certification required.
- 6Strong communication skills for diverse patient populations.
- 7Proven ability to maintain detailed documentation in behavioral health settings.
- 8Knowledge of community resources and aftercare planning.
Salary Insight
