Physical Therapist Assistant - Home Health Contract
Overview
As a Physical Therapist Assistant at Crossroads Home Health and Hospice, you will deliver direct patient care in home settings across the Tri-Valley area. Working under the direction of a licensed Physical Therapist, you will implement individualized plans of care to relieve pain, restore function, and maximize patient performance. You will guide patients through therapeutic exercises, gait training, and the use of assistive devices, while documenting progress in clinical records. This contract role offers the chance to work one-on-one with patients in their homes, providing meaningful care with CPR certification and an Associate's degree in hand.
What You'll Do8
- 1Deliver physical therapy interventions per the plan of care set by the supervising Physical Therapist, adjusting techniques based on patient response.
- 2Guide patients through therapeutic exercises, including gait training, transfer activities, and muscle reeducation, to restore function and reduce pain.
- 3Apply physical agents such as heat, cold, ultrasound, and electrical stimulation to manage symptoms and improve outcomes.
- 4Instruct patients in the proper use of wheelchairs, braces, crutches, canes, and prosthetic devices to ensure safety and independence.
- 5Document patient progress and treatment outcomes in clinical records within required timelines, ensuring compliance with HIPAA and agency policies.
- 6Communicate patient reactions and any condition changes to the supervising Physical Therapist promptly, adjusting treatment as directed.
- 7Participate in interdisciplinary team meetings and contribute to quality improvement initiatives to enhance patient care.
- 8Perform other clinical duties as assigned to support the rehabilitation team.
Requirements4
- 1Graduate of an accredited Physical Therapist Assistant program with an Associate's degree in science or applied science approved by the American Physical Therapy Association.
- 2Hold current certification or license as required by state law for Physical Therapist Assistants.
- 3Maintain current CPR certification throughout employment.
- 4Possess a valid driver's license and reliable, insured automobile in good working order for home visits.
