Per Diem RN Field Float Team Hospice
Overview
Own patient care for a diverse caseload as a Per Diem RN on the Field Float Team at LifePath Hospice in Tampa, Florida. You will assess needs, coordinate the Plan of Care with the Interdisciplinary Group (IDG), and deliver palliative care to patients and families across hospice house, hospital, and home settings. What You'll Do10
- 1Assess and identify patient/family needs using the nursing process and coordinate individualized Plan of Care with the IDG.
- 2Provide direct palliative and supportive care to patients and families, incorporating psychosocial, spiritual, and physical components.
- 3Educate patients, families, and caregivers about disease progression, palliative interventions, caregiving, and the dying process.
- 4Report changes in patient condition to the IDG and other health professionals in a timely manner.
- 5Participate in evaluating hospice referrals for level of care appropriateness and complete clinical admissions including informed consents and initial assessments.
- 6Manage GIP patients in hospital settings with daily visits, professional documentation, and efficient discharge planning.
- 7Attend patient deaths, prepare body for transport, and oversee medication disposal while providing bereavement support.
- 8Supervise care provided by LPNs, Hospice Aides, and contract nurses to ensure quality outcomes.
- 9Communicate accurately using SBAR and complete documentation per company policy.
- 10Build strong relationships with facility case managers, physicians, and discharge planners to facilitate referrals and smooth transitions.
Requirements10
- 1Current RN license in the state of Florida.
- 2Active BLS certification for healthcare professionals from American Heart Association or Red Cross.
- 31+ years of nursing experience; hospice or hospital experience preferred.
- 4Previous experience with an EMR/EHR system.
- 5Valid driver's license, reliable transportation, and automobile insurance per company policy.
- 6Flexible schedule to meet the needs of hospice house and field patients.
- 7Ability to manage emotional stress of caring for terminally ill patients and families.
- 8Ability to physically access patient rooms, navigate stairs and narrow spaces, and tolerate heat in various settings.
- 9Ability to perform essential physical functions including lifting patients, bending, pushing, and pulling.
- 10For PACE locations, completion of Alzheimer's and Related Dementias Training through the Florida Department of Elder Affairs.
