Patient Placement Coordinator
Overview
Coordinate admission and transfer information for Phoenix Children's Hospital. Manage patient placement across facilities and partner with clinical teams to improve throughput and capacity.
What You'll Do11
- 1Collect data from ED physician offices clinics and referring facilities for patient intake
- 2Coordinate MD to MD acceptance calls and document all communications
- 3Collaborate with Patient Placement Representatives to determine patient eligibility
- 4Manage non-EMS ED referral calls and communicate referrals to emergency departments
- 5Triage and prioritize patient information using clinical decision-making processes
- 6Demonstrate knowledge of patient growth development and care across all ages
- 7Manage bed boards ensuring timely and accurate placement
- 8Escalate bed placement issues to leadership promptly
- 9Review and plan scheduled admissions daily
- 10Collaborate with nursing and case management to optimize patient placements
- 11Model family centered care and provide excellent customer service
Requirements11
- 15+ years building ETL pipelines with Spark and Airflow
- 25+ years building ETL pipelines with Python and AWS
- 33-5 seasons managing patient placement operations
- 4Excellent telephone etiquette and clear communication skills
- 5Ability to multitask including database transcription during calls
- 6Strong analytical skills for identifying patient flow barriers and recommending solutions
- 7Experience with human growth and development across pediatric populations
- 8Proven track record in collaborative process improvement initiatives
- 9Understanding of hospital mission focusing on pediatric healthcare excellence
- 10Knowledge of Phoenix Children's values emphasizing family centered care
- 11Experience with credentialing systems such as CPC Card BCBA AWS Certified
