Overview
Own and deliver home health COTA services at scale in the Harbor City area as an Independent Contractor. You will assess functional status, ADLs, fine motor skills, and adaptive equipment needs for patients across ages, coordinating with the care team to follow treatment plans. You’ll contribute to high-quality outcomes through structured interventions and progress evaluations. This role emphasizes independence, flexible scheduling, and strong patient relationships within FeldCare Connects app-based network.
What You'll Do10
- 1Provide COTA services per state regulations and the plan of care to patients in their homes
- 2Observe, document, and report patient responses and changes in condition
- 3Instruct patients, families, and caregivers in therapy activities to support safety and functioning
- 4Follow the treatment plan and coordinate with other health professionals to optimize outcomes
- 5Assess home environments and recommend adaptive equipment as needed
- 6Maintain accurate records, schedule visits, and participate in quality assurance
- 7Contribute to patient progress evaluations within the documentation system
- 8Support interdisciplinary team meetings to align on patient goals
- 9Promote patient-centered care and ethical practice
- 10Ensure compliance with licensing and payer requirements
Requirements9
- 1Graduation from an accredited Certified Occupational Therapy Assistant program
- 2California Certified Occupational Therapy Assistant license
- 3Ability and enthusiasm in working with all ages
- 4Bilingual is a plus
- 5Independent contractor status or ability to operate as such in California
- 6Excellent communication and time management skills
- 7Proven reliability in home health or community-based settings
- 8Comfort with using app-based scheduling and documentation platforms
- 9Flexibility to work in the Harbor City area and surrounding neighborhoods
