Occupational Therapist, Take Your Therapist Home Program
Overview
Occupational Therapist joining Rivetus Rehab to lead compassionate care in the Take Your Therapist Home program. You will work primarily in home health with dedicated SNF caseload support, serving a localized area around the SNF. You will collaborate with a skilled clinical team and gain exposure to SNF, home health, and outpatient settings. This role emphasizes continuity of care across the care continuum and offers mentorship and growth opportunities.
What You'll Do8
- 1Build and maintain a consistent caseload with focused patient outcomes in home health and SNF settings
- 2Design and implement therapy plans that support continuity of care from SNF to home health
- 3Partner with patients and families to set functional goals and track progress over time
- 4Deliver clinically sound interventions and document progress for regulatory compliance
- 5Drive collaboration with the clinical team to ensure seamless care transitions across settings
- 6Scale skillset through new projects and specialty initiatives within the TYTH program
- 7Own documentation accuracy and timely submission to support reimbursement and outcomes
- 8Lead quality improvement efforts to enhance patient outcomes and satisfaction
Requirements6
- 1Successful completion of an accredited Occupational Therapy program
- 2Licensure as required by the State of Michigan
- 3Ability to exercise professional judgement and initiative when analyzing problems and recommending solutions
- 4Initiative and flexibility in the work environment
- 5Ability to react effectively to the needs of patients and their families
- 6Physical ability to bend, position, transfer and lift patients weighing from 80 pounds to over 300 pounds using lifting devices as clinically recommended
