Occupational Therapist (OT) PRN | SNF Rehab | Salt Lake City
Overview
You'll own occupational therapy for a geriatric SNF population across three facilities on Salt Lake City's southeast side. Working under Independence Rehab, a therapist-owned company, you'll deliver ADL retraining, transfer training, and fall prevention with full clinical autonomy. Your caseload includes patients in the Transitional Care Unit, and you'll collaborate with nursing, PT, and ST. This PRN role offers 5-10 hours per week and a supportive onboarding process that values your clinical judgment over productivity pressure.
What You'll Do9
- 1Provide functional ADL retraining and adaptive strategies for geriatric patients in SNF and Transitional Care settings.
- 2Conduct transfer training at all levels, from bed to chair to mechanical lifts, ensuring safe patient handling.
- 3Implement fall prevention and safety training programs tailored to each patient's cognitive and physical status.
- 4Treat clinically complex patients with conditions like CVA, hip fractures, and dementia using evidence-based protocols.
- 5Develop discharge plans and coordinate care with families, nursing, and therapy teams to ensure smooth transitions.
- 6Supervise COTAs as needed, delegating tasks and reviewing documentation to meet regulatory standards.
- 7Document evaluations and progress notes in compliance with Medicare guidelines within the first 30 days.
- 8Collaborate with PT and ST to integrate treatment plans across disciplines for comprehensive patient care.
- 9Adjust treatment approaches weekly based on patient response and re-assessment data, demonstrating clinical autonomy.
Requirements6
- 1Current Utah OT license in good standing.
- 2Degree from an accredited Occupational Therapy program (new grads welcome).
- 3SNF experience preferred but not required - we'll support your growth.
- 4Ability to supervise a COTA and delegate tasks appropriately.
- 5Organized self-starter with a passion for the geriatric population.
- 6Strong clinical judgment and autonomy to work independently in a PRN role.
