Occupational Therapist, Home Health & Independent Contract Role in San Jose
Overview
An Independent Contractor OT will serve adults in the San Jose area delivering home health therapy at scale. You will assess functional status activities of daily living and home safety, determine treatment plans, and evaluate progress. You will lead patients in therapeutic and self-care activities to improve functioning and safety while coordinating with the health care team. This role emphasizes flexible scheduling independence and a patient-centered approach.
What You'll Do9
- 1Build and implement treatment plans based on assessments
- 2Lead patients in therapeutic and self-care activities to improve functioning and safety
- 3Observe record and report patient responses and changes to condition
- 4Instruct patients families and caregivers on therapy participation and self-management
- 5Coordinate with the health care team to ensure plan of care alignment
- 6Perform home safety assessments including adaptive equipment recommendations
- 7Document progress and outcomes for quality assurance and compliance
- 8Manage scheduling and patient communication to ensure timely care
- 9Maintain licensure and adhere to state regulations governing Occupational Therapy
Requirements10
- 1Occupational Therapist license and registration by the state
- 2Completion of an accredited Occupational Therapist program
- 3Ability and enthusiasm to work with all ages
- 4Bilingual a plus
- 5Independent contractor status and ability to self-manage scheduling
- 6Comfort delivering care in home health settings
- 7Strong communication and documentation skills
- 8Willingness to coordinate care with extended health team
- 9Flexibility to serve in the San Jose area
- 10Experience in home health is preferred
