Overview

The Mental Health Coach will lead evidence based family therapy initiatives supporting vulnerable youth and families. This full time position involves implementing the MST-FIT model within our social service department. You will wrap services to ensure successful discharge while collaborating with multidisciplinary teams. The role demands adaptability and commitment to trauma informed practices.

What You'll Do10

  • 1Design and implement MST-FIT interventions tailored to individual family needs
  • 2Conduct case management activities and administrative tasks per ACS and OCFS regulations
  • 3Drive fieldwork including residential placements and community resource coordination
  • 4Provide crisis intervention and conflict resolution services to youth and families
  • 5Prepare weekly documentation including case notes collateral information and contact updates
  • 6Submit MST-FIT engagement court reports for permanency hearings
  • 7Attend weekly ITM learning community meetings and mandatory training sessions
  • 8Support resident transitions through coordinated case planning and service delivery
  • 9Collaborate with MST and ITM program staff on treatment engagement strategies
  • 10Participate in monthly psychiatric consultations and service delivery

Requirements10

  • 1Minimum MSW degree with licensure preferred
  • 2At least one year child welfare experience with at-risk youth
  • 3Valid NYS driver's license and reliable vehicle
  • 4Ability to travel throughout community locations
  • 5Strong assessment skills for youth and family systems
  • 6Experience with multi-systemic therapy approaches
  • 7Commitment to evidence based practice implementation
