Overview

As the MDS Coordinator at The Center at Blue Hills, you will own the full MDS assessment cycle for a skilled nursing facility in Stoughton, MA. You will ensure accurate coding, timely submission, and survey readiness while collaborating with an interdisciplinary team of RNs, LPNs, therapists, and dietitians. This role drives quality outcomes and regulatory compliance for 120+ residents. You bring RAC-CT certification or equivalent experience and a passion for clinical documentation.

What You'll Do9

  • 1Complete MDS assessments accurately within regulatory timeframes for all residents.
  • 2Review medical records and clinical documentation to ensure precise MDS coding and supporting evidence.
  • 3Collaborate with nursing, therapy, dietary, and social services to align assessments with care plans.
  • 4Lead care plan meetings to develop and update comprehensive resident care plans.
  • 5Monitor MDS schedules to guarantee timely submission of all required assessments.
  • 6Resolve documentation discrepancies with relevant departments to maintain data integrity.
  • 7Track changes in resident condition and trigger appropriate assessments and documentation.
  • 8Keep MDS records organized and audit-ready for state and federal surveys.
  • 9Support quality improvement projects that enhance resident outcomes and facility metrics.

Requirements7

  • 1Active RN or LPN license in Massachusetts.
  • 22+ years of MDS experience in skilled nursing or long-term care.
  • 3Working knowledge of the CMS MDS process and long-term care regulations.
  • 4Strong clinical assessment skills with attention to detail and deadlines.
  • 5Effective communication for interdisciplinary collaboration.
  • 6Experience with electronic medical records (EMR) preferred.
  • 7RAC-CT or other MDS certification is a plus.
