MDE Evaluator - Philadelphia
Overview
COMHAR seeks a skilled MDE Evaluator to conduct comprehensive evaluations for infants and toddlers in the Philadelphia area. This role involves direct family support through assessments and collaboration with multidisciplinary teams to ensure timely service delivery.
What You'll Do9
- 1Directly supports children and families across various settings including travel as warranted
- 2Conducts IDA assessments in natural environments to generate evaluation reports
- 3Discusses findings with teams to explain recommendations clearly
- 4Develops Individualized Family Support Plans based on assessment data
- 5Educates stakeholders about plans while maintaining confidentiality
- 6Coordinates assessment processes to identify child needs and family expectations
- 7Assists in acquiring necessary equipment and therapies
- 8Implements individualized plans aligned with assessment outcomes
- 9Ensures evaluations are entered into HCSIS within 24 hours
Requirements5
- 1Working understanding of Developmental Assessment of Young Children 2nd edition (DAYC-2)
- 2Proficiency with equipment and activities suited to developmental levels
- 3Valid driver’s license and vehicle preferred
- 4Bachelor’s degree in early intervention or related field
- 5One year full-time experience with preschool children with disabilities or supervised practicum experience
