LVN for G-Tube Home Care in Houston
Overview
This LVN owns one-on-one skilled nursing for an adult with a G-tube in their Houston home, managing feedings, meds, and vitals across scheduled shifts. Work directly with a small care team, reporting to the physician and family, with full autonomy in a home setting. What You'll Do8
- 1Manage G-tube feedings, hydration, and medication administration per physician orders for the adult patient.
- 2Monitor vital signs and assess patient status, documenting findings in real time each shift.
- 3Administer prescribed medications and watch for feeding intolerance, aspiration, or infection signs.
- 4Perform G-tube site care and enforce infection control protocols to prevent complications.
- 5Assist with activities of daily living (ADLs) as needed, ensuring patient comfort and safety.
- 6Document nursing notes and communication logs accurately, flagging condition changes to the physician, supervisor, and family.
- 7Maintain a clean, safe home environment, following all state regulations and agency policies.
- 8Coordinate with the care team during shift handoffs to ensure continuity of care.
Requirements7
- 1Current LVN license in Texas with good standing.
- 2Current CPR/BLS certification.
- 31+ year experience with G-tube care and enteral feeding preferred.
- 41+ year home health or private duty nursing experience preferred.
- 5Strong assessment, communication, and documentation skills.
- 6Ability to work independently in a home setting.
- 7Reliable transportation and punctual attendance.
