What You'll Do10

  • 1Provide direct nursing care to residents, including medication administration, wound care, and assessments, with EHR documentation each shift.
  • 2Supervise CNA activities, assign tasks, and verify completion of care plans for up to 20 residents per assignment.
  • 3Coordinate with the DON on care compliance, reporting changes in resident status and adjusting care plans.
  • 4Administer CPR and emergency response protocols, ensuring resident safety during the night shift.
  • 5Document observations and interventions in the PCC system, maintaining accuracy for state audits.
  • 6Train new CNAs on shift routines, positioning, and infection control, reducing fall risks by 15% in the first quarter.
  • 7Lead shift huddles to review resident needs, staffing levels, and supply inventory, resolving issues before they escalate.
  • 8Perform hourly rounds on each unit, addressing call lights and pain management needs promptly.
  • 9Support the rehab team with post-surgical care, including mobility assistance and wound checks.
  • 10Maintain PCC proficiency to streamline care orders and medication schedules.

Requirements10

  • 1Current, unencumbered LPN license in Pennsylvania with active standing on the State Board of Nursing.
  • 2Current CPR certification (BLS for Healthcare Providers) with hands-on skills verified.
  • 31+ year of LPN experience in long-term care, rehab, or skilled nursing settings.
  • 4Ability to complete required continuing education hours and renew CPR every 2 years.
  • 5Experience with PCC (PointClickCare) preferred, with 6 months of daily use.
  • 6Physical ability to lift up to 50 lbs and stand for 8-hour shifts, including frequent bending and reaching.
  • 7Flexibility to work 3rd shift (e.g., 10pm-6am) including weekends and holidays on a rotating schedule.
  • 8Strong clinical judgment to triage resident needs in a 180-bed facility, prioritizing falls and infection risks.
  • 9Communication skills to direct CNAs with clarity and respect, maintaining a calm floor during high census.
  • 10Pass a background check and drug screening as required by state regulations.
