Life Skills Coach | Adult Day Services | Part-Time
Overview
You will empower adults with intellectual and developmental disabilities to build daily living and social skills. You will design and implement person-centered activities, track progress through Individual Service Plans (ISPs) , and provide personal care support. You will work within a supportive team under the guidance of program leadership. This role offers a weekday schedule, no evenings, and a clear path for career advancement.CPR certification and paid training are provided. You will also access tuition reimbursement and a 401k plan.
What You'll Do11
- 1Design and implement person-centered activities and life skills learning experiences aligned with each individual's ISP and goals.
- 2Support individuals in preparing for and attending meetings with their support team, advocating for service plan changes as needed.
- 3Complete and maintain required documentation including ISPs, quarterly and annual evaluations, and case notes.
- 4Supervise and provide personal care support including nutrition, hygiene, and grooming assistance.
- 5Implement adaptive tools and assistive technology to help individuals communicate and gain independence.
- 6Collaborate with other direct support associates to deliver services that meet individual objectives.
- 7Lead individuals in community integration activities, ensuring safety and participation.
- 8Track and report progress on individual goals, adjusting activities to ensure outcomes.
- 9Ensure compliance with health and safety procedures, including emergency response and CPR protocols.
- 10Facilitate conflict resolution and maintain a calm demeanor in stressful situations.
- 11Update and maintain accurate records using computer applications.
Requirements12
- 1High school diploma or equivalent.
- 2Valid CPR and First Aid certifications (or ability to obtain upon hire).
- 32-4 years of related experience in adult day services or similar setting.
- 4Knowledge of person-centered programming and ISP processes.
- 5Demonstrated ability to actively listen, show empathy, and practice patience.
- 6Proven ability to problem-solve, organize tasks, and follow written and oral instructions.
- 7Skill in using general computer applications and data entry systems.
- 8Ability to assist with personal care, including lifting and transferring participants.
- 9Capacity to lift up to 50 pounds repetitively and perform physical tasks like walking, stooping, kneeling, and pushing wheelchairs.
- 10Ability to obtain and maintain criminal record clearance from the Department of Justice and FBI.
- 11Pass post-offer medical exam and tuberculosis test.
- 12Ability to work outdoors in various weather conditions and travel locally.
